It was lovely to see my patent over 6 months since her lower eyelid surgery. She disliked the laxity, wrinkles and increasing hollowness under her eyes. We discussed various approaches and ended up proceeding with a lower blepharoplasty with combined midfacelift.

Her primary concerns were a lack of breast volume and poor shape. As a strong athletic woman she had always felt small-breasted in comparison to her frame. To generate a natural defined shape extra-high projection anatomically-shaped (“teardrop”) implants were used. These must be used carefully so as not to create an unnatural degree of projection (how far forward the breast extends). In this case it was a perfect choice and delivered the shape change desired.

Her primary concerns were her marked dorsal hump and the large size of her nose. Morphing her initial photograph gave a surgical guide of what to aim for. She wanted to turn over a new leaf before starting university – so a date was found after her A’levels were finished. An ultrasonic Piezo technique enabled reduction and reconstruction of the bony skeleton with a strong stable result. In this case the morphed plan and the final result are nearly identical.

I am very grateful to my patient for allowing me to share these images. Her primary concerns were her loose neck and lower facial skin and poor neck and jawline definition. Whilst central neck access would be reasonable, in this case a tailored less aggressive approach was chosen by the patient, aiming to maximise a natural result which limited risk, downtime and cost.

My patient came to discuss breast augmentation – many of her family members had undergone this surgery and she wanted to regain the breast volume lost after three pregnancies. However she was a type 1 diabetic and used her abdominal tissue for injecting insulin. This was clearly an area that could be transformed by surgery and which would help with body confidence and clothing choices. A lot of work went into optimising her diabetic control and ensuring surgery could be undertaken in a safe way. Abdominoplasty and breast augmentation was undertaken together with no surgical complications and no problems with diabetic control. The goal of looking fabulous at her friend’s wedding later that year came true!

My lovely patient was very troubled by the large, indented and puckered scar on her abdominal wall. This was from an appendicectomy in childhood with subsequent wound infection and problematic healing. As a bikini enthusiast she hated this scar and sought surgery to correct it. Scars result from any injury to the skin (surgical, infective, traumatic or otherwise) and are permanent, inevitable and unpredictable. No non-surgical methods would have improved this scar significantly so by employing cosmetic techniques for reconstructive benefit, abdominoplasty surgery was proposed as a way to swap the puckered scar for standard fine abdominoplasty scars.

I am very grateful to my patient for allowing me to share these images. Her primary concerns were ageing of her lower face – particularly her jowls and her neck contour. Together we also identified that her upper lip had relatively lengthened and that shortening it very slightly would help her regain a more youthful appearance.
