Nasal asymmetry correction
As a mountain-climbing extreme athlete this 21 year-old man struggled to breathe through his nose upon exercise. He had significant nasal deviation and whilst there were other facial asymmetries present, this was what bothered him most. Careful discussion identified that functional concerns were at the top of his wishlist; climbing is his true passion and helping his ability in this extreme sport was the primary goal of surgery. Correction of nasal asymmetry of this degree is challenging. It requires re-positioning and centralisation of not only the nasal bones and the whole septum but also correction of asymmetrical nasal tip cartilage dimensions. This was undertaken without other graft material from elsewhere; some recycled septal bone was used to hold the straightened septum straight. Unless full and extensive release and repositioning is undertaken significant asymmetry may recur. In this case, whilst not a perfect centralisation, the result two years after surgery exceeded the patient’s expectations in both functional and appearance domains. In early 2023 he was the highest man on the planet – having summited Aconcagua in Argentina – the highest mountain outside of the Himalayas with a summit elevation of 6,961 metres.
