Nose surgery, or nasal surgery, is a generic term that covers a range of facial plastic surgery procedures around the nasal area. From rhinoplasty surgery to reshape the nose to full nasal reconstruction to reform damaged tissue and reconstruct the nasal cavities, there are many different options.

The right option for you will depend on your current nose and your aesthetic goals. As an experienced nose surgeon, Sinclair Gore is able to carry out all types of nose surgery, ranging from preservation rhinoplasty to full nasal reconstruction to completely rebuild the nose.

To help you understand the different options available and how to prepare for nose surgery, Sinclair Gore has put together this expert guide.

Reasons You Might Need Nasal Surgery

There are several reasons why you might need nose surgery, including:

  • Dissatisfaction: If you simply don’t like your current nose shape, then cosmetic rhinoplasty, also known as a nose job, could help you get the look you desire and feel more confident.
  • Revision Surgery: Past nose surgery carried out by other surgeons could lead to a look you’re still unhappy with, meaning revision rhinoplasty surgery might be needed to fix the issue.
  • Developmental issues: Some people simply grow up with a nose that is unusually large or small
  • Breathing Difficulties: Issues such as a deviated septum or sinus problems can cause breathing problems, which might require surgery to improve nasal breathing.
  • Previous Drug Use: Some recreational drugs, such as cocaine, can erode the septum and cause nasal disfigurement.
  • Accidents: Incidents such as dog bites or car accidents could lead to anything from a broken nose that heals crookedly to large areas of missing flesh, which could require full nasal reconstruction to repair the damage.
  • Skin cancer: It is common for skin cancer surgery on the nose to leave the nose asymmetrical, misshapen or with sections completely missing. Nasal reconstruction is an advanced form of reconstructive surgery that rebuilds sections of the nose.

Every patient is different, and some might require medical or cosmetic nose surgery. Whatever type of nose surgery you need, there is an option for you, and expert surgeons like Sinclair Gore can help identify the type of nose surgery needed.

Common Types Of Nose Surgery

There are many different types of nose surgery available, depending on why you need surgery, and the outcome you expect. Some of the most common options include:

Rhinoplasty

Rhinoplasty surgery is a term used for a range of cosmetic and medical procedures designed to reshape the nose. The aim is to modify and rearrange the skin, cartilage and bone across the nose to reshape it, allowing for a new look or to improve airflow.

When carrying out rhinoplasty, your surgeon will access the nasal structures through an incision within the nasal passages. They can then remodel the cartilage and bone to reshape the nasal tip, bridge or nostrils.

There are many different types of rhinoplasty, depending on the approach taken and the main goal. These include:

  • Closed Rhinoplasty: With this technique the incisions are placed inside the nostrils, with much of the surgery undertaken blind rather than under direct vision.
  • Open Rhinoplasty: This is an extended technique beyond closed rhinoplasty, where the internal nostrils incisions are connected by a small zigzag incision on the columella (between the nostrils), enabling all the structures of the nose to be seen and operated on. This permits a wider range of techniques than in closed rhinoplasty.
  • Preservation rhinoplasty: This modern concept aims to preserve certain key structures, including the smooth natural bony dorsum of the nose, and re-fashion other areas such that the final result is as you want it. It aims to avoid unnatural contour irregularities and divots, which can result from traditional older techniques.
  • Reduction Rhinoplasty: This option aims to make the nose smaller by reducing a dorsal hump and/or shortening your nasal structure.
  • Augmentation Rhinoplasty: An alternative to reduction rhinoplasty, augmentation rhinoplasty makes the nose larger using cartilage grafts and other methods.
  • Revision Rhinoplasty: Also known as secondary rhinoplasty, this procedure aims to improve on nasal shape and function after previous surgery.
  • Tip Plasty: For changes in the tip only, a tip plasty involves a focus on the tip of the nose without any procedures on the bridge of the dorsum of the nose.

These are just some of the rhinoplasty options available. Sinclair Gore is a proponent of ‘preservation rhinoplasty’, also known as reconstructive rhinoplasty, which is a modern rhinoplasty procedure that aims to produce a smooth, natural nose appearance by preserving key components of the nasal bridge bone and cartilages.

Septal Surgery

Septal surgery, sometimes referred to as Septoplasty, is a procedure to fix a deviated nasal septum or correct an internal obstruction.

The septum is a thin wall of tissue that provides structural support for the nose. Most patients who require septal surgery have a deviated septum, which is where the septum is displaced and not centred, creating uneven nasal passages that can obstruct breathing and create an asymmetrical appearance.

Septal surgery is performed under general aesthetic and often involves just an incision inside the nostrils, meaning there won’t be external scars. Septoplasty can be cosmetic surgery to straighten the tip of the nose or for medical reasons, such as to reduce the symptoms of nasal obstruction. If combined with rhinoplasty surgery to alter the shape of the external nose, a combined procedure is known as septorhinoplasty, which is commonly performed by Sinclair Gore.

Nasal Reconstruction

Nasal reconstruction is the most intensive of all the nasal surgery options listed here, and is designed to completely reconstruct the nose. It often involves moving tissue from elsewhere in the body, including skin grafts and flaps, and cartilage from other parts of the body. By this means the new nose area can be built up with lining, carriage structure and surface skin covering to masquerade a normal nose.

This procedure is highly complex, and often includes multiple stages to ensure the correct outcome. Only highly experienced surgeons, like Sinclair Gore, have the skills and knowledge needed to carry out full nasal reconstruction.

One method of nasal reconstruction, which is highly advanced, is a staged paramedian forehead flap, which aims to providing a reliable, vascularised tissue match to restore missing or heavily damaged noses.

The process uses the supratrochlear artery to nourish a skin flap from the forehead, which is then transposed to the nose across multiple stages to allow for healing, sculpting, and final contouring to create a new nose.

How To Choose The Right Nose Surgery Option For You

With so many surgical techniques available, it can be daunting to work out which one is right for you. Thankfully, when you work with a skilled surgeon with extensive experience in nose surgery, like Sinclair Gore, you can make the most of their expertise.
